Physician- Primary Care

Location: Gateway Community Health Centre, 41 McClellan Street, Tweed, Ontario, Canada.

Job Type: Full-time, Permanent (New Position).

Salary: $331,974 annually.

Schedule: Monday to Friday, 37.5‑hour work week.

Gateway Community Health Centre is a rural Ontario primary care provider that delivers comprehensive care, focusing particularly on those at higher risk or facing barriers to access. The centre promotes health, prevents illness, and provides allied health and dental services through a collaborative inter‑professional team.

We are seeking a Primary Care Physician to join our team. Physicians maintain a roster of patients while working closely with allied health professionals, a dental clinic, and the broader community health team to deliver high‑quality, coordinated care. The approach blends medical treatment with an understanding of social determinants of health, and utilizes electronic medical records and digital tools to support effective communication across the team.

• Maintain a roster of primary‑care patients.
• Deliver patient‑centred primary care within a collaborative, inter‑professional team.
• Coordinate care with allied health professionals and the dental clinic.
• Use electronic medical records and digital tools to support effective communication and continuity of care.
• Engage in continuous professional development and apply best practices in health promotion and illness prevention.
